Common problems & what to know before you buy
Before you message the seller, know the documented failure patterns, repair cost exposure, and risk level for the 1991 GMC Sierra.
What to watch for on the 1991 GMC Sierra
Structural and Body Rust
Cab corners, rocker panels, bed sides, and frame rails around the rear spring and cab mounts. On a truck this age rust is the deciding factor, and bodywork frequently costs more than the vehicle is worth.
4L60E Transmission Failure
The four-speed automatic is the weakest part of the drivetrain, particularly behind the V8s and when used for towing. Slipping, flaring, or a harsh 1-2 shift means a rebuild is due.

1991 GMC Sierra price ranges
Clean, rust-free examples have appreciated noticeably as these trucks enter enthusiast territory, particularly single-cab short-bed versions. Rusty work trucks remain very cheap. The 1996 switch to Vortec heads brought a worthwhile power increase and is the version to prefer.

GMC Sierra (GMT400) Buyer's Guide
Simple, Cheap, and Rusting
The GMT400 Sierra, built from 1988 through 1998, is the last genuinely simple full-size GM pickup and remains a staple work truck and a growing enthusiast favourite. Small-block V8s, a straightforward chassis, and universal parts availability make it one of the easiest vehicles to keep on the road that you can buy.
Engines: The 5.7-liter 350 is the volume choice, throttle-body injected early and Vortec from 1996 with a useful power increase. A 4.3-liter V6 covers base trucks, the 7.4-liter 454 handles heavy work, and the 6.5-liter diesel exists but brings its own problems, principally the injection pump driver module that fails with heat.
What to look for: Rust decides value. Check cab corners, rocker panels, bed sides, and the frame, particularly around the rear spring mounts and where the cab mounts sit. Then check the transmission: the 4L60E is the weak point of the drivetrain and a slipping or flaring shift means a rebuild.
Reality check: Nothing about this truck is refined, but almost nothing about it is expensive either. Parts are cheap and everywhere, the mechanicals are simple enough for home repair, and knowledge is abundant. Buy the least rusty one you can find and expect to do maintenance rather than repairs.
About the GMT400
The GMT400 GMC Sierra, produced from 1988 through 1998, is the last genuinely simple full-size GM pickup and has become both a staple work truck and a growing enthusiast favourite. The 5.7-liter 350 small block is the volume engine, throttle-body injected early and switching to the more powerful Vortec configuration in 1996, with a 4.3-liter V6 below it, a 7.4-liter 454 for heavy work, and a 6.5-liter diesel option. Everything about the chassis and drivetrain is straightforward, parts are universally available and cheap, and most maintenance is achievable at home.
Rust is what decides value on these trucks. Cab corners, rocker panels, bed sides, and the frame around the rear spring and cab mounts all corrode, and bodywork routinely costs more than the vehicle is worth. Mechanically the weak point is the 4L60E four-speed automatic, particularly behind a V8 and when towing, where slipping or flaring shifts signal a rebuild. On diesel trucks the pump-mounted injection driver module fails with heat and causes intermittent stalling, a well-known fault with an equally well-known relocation fix. Clean, rust-free examples, especially short-bed regular cabs, have appreciated noticeably as these trucks enter collector territory.

Frequently asked questions
Is the 4L60E really that fragile?
It is the weakest link in an otherwise durable drivetrain, especially behind a V8 and when towing. Rebuilds are common and relatively affordable, and upgraded parts are widely available. Treat a slipping transmission as a negotiating point rather than a dealbreaker.
TBI or Vortec 350?
The 1996-on Vortec is meaningfully more powerful and more efficient, and is the better engine. The earlier throttle-body version is simpler and marginally easier to work on. Both are durable; the Vortec is the one to pay a little more for.
Are these appreciating?
Clean examples are, particularly short-bed regular cabs and well-optioned SLT trucks. Rusty ones are not and will not. As with most trucks of this era, condition rather than specification is what the market rewards.
